David Cassidy has been hospitalized for liver and kidney failure and is now in critical condition, his rep confirms to PEOPLE. Page Six reports that Cassidy's health is rapidly deteriorating, but that he is "conscious now and surrounded by family".

Cassidy, in an appearance on the talk show "Dr. Phil" earlier this year, opened up about the diagnosis, saying his mother and grandfather also both suffered from dementia.

"He is hoping and waiting for a liver transplant, if he happens to get one in time, the prognosis is still up in the air", the source told Us.
